Lawrence Eye Care accepts most insurance plans, click here for more information. We encourage you to contact your insurance provider and determine your coverage prior to your initial visit. In addition, our billing office is available to answer any questions you may have. Please call (785) 841-2280 for assistance.
When you call to schedule an appointment, please have the following patient information ready:
Full name, including middle initial
Your current phone number
Your birthdate
The name and phone number for your referring doctor and primary care physician
Your insurance card, along with the name and birth date of the subscriber
The nature or reason for your call and purpose of the appointment
*NEW PATIENTS*
Before scheduling an appointment, our physicians need records from your previous eye care providers or referring physicians. You can access the record request form by clicking here. Once we receive and review your records, we'll schedule your appointment. At that time, you can fill out the new patient paperwork online here, have it mailed to you, or pick it up from our office. Please ensure all paperwork is completed and submitted before your appointment, or arrive 15 minutes early to complete the necessary forms.
*EXISTING PATIENTS*

To make your experience at Lawrence Eye Care go smoothly, please bring the following information with you when you arrive for your appointment:
Insurance cards
Photo ID
Your copayment and coinsurance amounts. We require payment of these at the time of service
Completed Patient Information forms that you will receive prior to your visit (unless you already submitted that information online or via mail).
List of any current medications(s) (how much you take and how often) and/or drug allergies (unless you already submitted that information online or via mail).
Change of address/phone number(s) if you have recently moved or made a change
Emergency contact and their phone number (required)
Most surgical procedures require a separate visit in addition to the clinic visit
